If you are a CSUSM Employee and have a necessary mission-critical reason to access CSUSM or CSUSM@Temecula buildings and/or facilities, please complete and submit this request form.  This form is for single business purposes only.  If your request is intended for consistent daily, weekly, etc. campus access, please exit this form and speak to your MPP or Dean about a Return to Campus repopulation process form.  


Safety, Health & Sustainability will inform you if your request is approved.  
  

  • Access to facilities and buildings at both campuses is restricted to essential personnel only at this time
  • If you are scheduled to teach “face to face” on campus, you do not need to fill this form out, but must complete the daily symptom form 
  • Approval is required from a member of the President's Administrative Team or their delegate 
  • Please ensure your selection of the division/college is correct so your request is not delayed or denied 
  • Safety, Health & Sustainability is managing the process in partnership with University Police Department to ensure an accurate record of people on campus.  They do not have access to your request until it has been properly routed and approved  \
 

All employees, CSUSM auxiliaries, visitors, contractors and vendors who are granted access are expected to follow the requirements official health orders issued by the applicable country Public Health Officer (San Diego County or Riverside County).
