Hello! Welcome! Thank you for participating in the Lactation Room Utilization Survey. These questions are for a research project concerning the university community's usage of the lactation spaces on campus. This survey is conducted by a public health faculty and their team of public health graduate students and alumnus from California State University San Marcos (CSUSM). Our project has been fully approved by CSUSM Institutional Review Board. This survey will take approximately 15 minutes. 
 
You can answer either on your computer or on your phone. 
 
We are interested in your experience using the lactation spaces on campus. The first section is to be answered during each lactation room visit and will be regarding room use – there are twelve questions in this first section. The second section is to be answered on the first visit, and then every three months after – these are general demographic questions – there are up to 12 questions in this second section.
 
Responses are completely anonymous and confidential, with an option at the end to participate in the monthly incentive giveaway by providing your e-mail address (optional). Your response will help us evaluate, improve and advocate for the lactation accommodations on the campus in order to better meet needs of the population. There are no identified risks associated with participation in this survey.
 
Participation in this survey is completely voluntary and you may stop participating at any time for any reason. If you do need to stop, be sure to save, and you can come back later and complete the survey. By participating in this survey, you are indicating you understand your responses are anonymous and will not be identified with you in any way and that you are at least 18 years old.
