Invitation to Participate
Dr. Kim Pulvers, a researcher at California State University San Marcos, is conducting a study on the relationship between health characteristics, behaviors, stress, and cognition. You are invited to participate as an adult at least 18 years of age or older.
Purpose
This study is being done to understand different health characteristics and behaviors among college-aged students.

Description of Procedures 

This study is divided into two parts. The first part is an online survey about health and behaviors which will take approximately fifteen to twenty minutes to complete. The survey contains questions about your background such as age, experience with issues such as stress, health behaviors such as exercise and marijuana use, family medical history, and medical history questions to ensure the study is safe for you.

The online survey is being conducted to identify participants who may be eligible to complete an in-person study. The in-person study will take 90 minutes and be worth three HPP credits (or alternate compensation if you’re not an HPP participant). Your name and contact information will be asked at the end of the survey for the sole purpose of inviting you to participate in the in-person study if you’re eligible. Immediately after checking your eligibility for the in-person study, your survey responses will be assigned a code number and your name and contact information will be deleted. The online survey will be administered through a secure web-hosted platform, survey responses are accessible only by password, and only trained research staff will have the password.

Risks and Inconveniences

1) The amount of time for the study may be inconvenient.

2) There is a low risk of information being seen by others.

3) The content of the questionnaires may cause minimal discomfort.

Safeguards

1) You will be compensated for your time with HPP credit or other arrangements (i.e., extra credit, gift card)

2) The online survey will be administered through a secure web-hosted platform, survey responses are accessible only by password, and only trained research staff will have the password. Once your responses to certain questions are checked to determine eligibility for the in-person study, your survey responses will be assigned a code number and your name and contact information will be deleted.

3) You may choose not to answer questions and can discontinue at any time.

Benefits

There is no direct benefit to you for participating in this study.  However, this study will help us learn about health behaviors among a college population. By participating in this study, you will be informing research of college students’ health and behaviors. 

Incentives

This survey study is worth up to 1 credit upon completion (one credit per half hour) if you are an HPP participant, or extra credit or gift card opportunity drawing if non-HPP participant. If you choose to discontinue the survey, you will still receive your HPP credits.

Confidentiality

Your survey responses will be kept confidential and will only be available to the research team for analysis purposes. Survey responses are accessible only by password, and only trained research staff will have the password. Once your responses to certain questions are checked to determine eligibility for the in-person study, your survey responses will be assigned a code number and your name and contact information will be deleted. 

Voluntary Participation

Participation is voluntary.  You do not have to participate in this study if you do not want to.  If you agree to be in this study, but later change your mind, you may withdraw at any time. You may take more time to consider your desire to participate in this study and complete the survey at a later date.
